Medical certificate renewal is a separate workflow

A USCG medical certificate is not the same document as an MMC, and renewing one is not automatically the same task as renewing the other. The current NMC Medical Certificate page organizes medical applications, forms, status help, duplicate requests, and additional-information submissions.

Start there when the question is about the medical certificate. Use the general MMC renewal guide when the question is about the credential application itself. Keeping these workflows separate prevents a medical document from being sent through the wrong channel.

Check expiration before choosing a transaction

Find the expiration date on your medical certificate and identify what you are asking the NMC to do. A renewal, a duplicate, a response to an existing application, and a new certificate can require different instructions. Do not select a form based only on the fact that your previous certificate used it.

Create a small renewal folder containing the current certificate, prior correspondence, reference number, submitted forms, medical-provider documents, and confirmation details. Record the date you checked the NMC page so you can refresh the instructions before submission.

CG-719K and CG-719K/E

The NMC identifies CG-719K as the Application for Merchant Mariner Medical Certificate and CG-719K/E as the “Entry Level Only” application. The correct form depends on the medical certificate and credential situation you are addressing.

The NMC currently instructs applicants not to complete CG-719K in the web browser. Save the file, use an appropriate Adobe PDF application, and preserve the completed copy. Before applying, confirm that the PDF opens after saving and that the information is legible.

  • Does the form match the transaction and certificate type?
  • Are your name and reference details consistent?
